Migrate Match Card Templates
Add match card templates with club subs + playing orders to existing leagues
What this migration does:
- Creates default match card template for each league's sport
- Enables club substitute system (players can play for other teams in same club)
- Adds playing order configurations for all 14 sports
- Skips leagues that already have templates
- Safe to run multiple times - won't duplicate
